Answer: If you wish to combine duplicate places, they must match exactly in all columns in the Master Place List. This includes the columns which are viewed by scrolling to the right with the scroll bar at the bottom of the Master Place List.
With the Master Place List closed, access File > Maintenance > Optimize to cause the program to combine the duplicate places.
